Baptism March 14th, 2012	Baptism at our Community Live! service, March at 7:00PM. As part of the registration process, we ask that you write a few paragraphs (Question 14 & 15) describing how and when you decided to trust Christ alone for your salvation. We feel this is helpful for you to recognize the difference Christ has made in your life. Parts of your story will be read the night of your baptism.

Facility Request	Use this form to send a request to our Facility staff to use the building.
Food and Beverage Service or Kitchen Use Request	Form for requesting any type of food or beverage service for your Woodcrest event or use of the kitchen by your volunteer team.
FPU Scholarship Request-Spring 2012	Please make sure you have registered for FPU Spring 2012 BEFORE completing this form. Woodcrest Chapel has limited funds available for families with a financial need. Please complete this application to be considered for a scholarship.

Life At Woodcrest - Spring 2012	Please choose below which class you would like to take.Life at Woodcrest is an opportunity to explore the meaning of life in a relaxed, friendly setting. This is where no question about life, culture, truth or God is seen as too simple or too hostile. Choose one of the classes below to begin your journey.
Life Group Request	Life groups meet throughout the week and all around the city. Help us find the right group for you by telling us a bit about yourself.
Parent's Night Out	Parents, enjoy a night on the town by dropping your kids infant-5th grade at Kidcrest from 6-10pm Friday, Feb.17th. Childcare provided by Woodcrest Student Ministry youth and parents. $25 for 1, $15/additional children. studentministry@woodcrest.org
Prayer Request	Submit a prayer request to our staff and prayer ministry.
Premarital Application Packet	Please fill this registration form out if you would like to be contacted and receive a Premarital packet which includes information on our upcoming classes and/or wedding services.
Premarital Registration Spring 2012	Before registering for this class, please make sure you have received a Premarital packet from our offices and have returned the application packet. Participation in this class does not guarantee that your marriage will be officiated by a Woodcrest pastor. Our 10 class begins on February 9th and meets on Thursday evenings from 7-9pm.

Solid Rock Day Camp Spring Break, 2012	A high energy sports day camp for 1st-6th graders for Spring Break 2012, March 26-30. Camp meets at The Edge 7:30-5:30. Multi children discounts. PLEASE START WITH PARENT/GUARDIAN INFORMATION THEN COMPLETE FOR YOUR STUDENT.
Solid Rock Day Camp, Summer 2012	Please start by registering adult's information and add your child later. Camp meets at the Edge. Daily rates are $30/day for 1 child and $50/day for 2 children. Weekly rates are $125/week for 1 child and $200 for 2 children.
Technical Services and Support Requests	Completion of this form is required (whether you need tech or not) in order to reserve the Main Auditorium, Gym, Studio, Jeff City Auditorium, Kidcrest Auditoriums and Underground.
